Virtual Lifetime Electronic Record

"Connect Your Docs" through the Virtual Lifetime Electronic Record (VLER), the Veteran's Health Information Exchange Program . This program gives VA and community health care providers secure access to certain parts of your electronic health record. This access reduces the need for Veterans and their families to request and carry paper medical records from one health care provider to another. It also provides other potential benefits to Veterans and their providers.

How to join?  Signing up is easy.  You may sign up online*, through the mail, or in person.  *To protect your information, please do not email completed forms back to the medical center.

Online:  Through ebenefits www.ebenefits.va.gov or My HealtheVet www.myhealth.va.gov 

By Mail:  Download and complete VA Form 10-0485 and mail to:  Salem VA Medical Center; 1970 Roanoke Blvd (136); Salem, VA  24153

In Person:  Visit the VLER Table in the Foyer of Building 143; My HealtheVet Connection, Clinic Check-in Desk, Release of Information, or your Community Based Outpatient Clinic.
