Primary Care

About the Program

Patients are primarily seen for routine preventive visits, routine scheduled follow-up of acute and/or chronic health problems, and/or referred by consult to specialty clinics. Patients are also seen on a walk-in/call-in basis as problems exacerbate or new ones develop. Teaching to promote health and prevent disease as well as involvement of the patient/or significant others in his/her healthcare is also central to the Primary Care environment. A telephone linked care program is available 24 hours with ongoing clinical interventions as needed by Registered Professional Nurses. The program is patient-centered and patient-driven.

The Primary Care Service Line provides routine, holistic care to enrolled outpatient veterans.  Primary Care Teams (PCTs) serve as the "gatekeeper" for the patient's medical needs. Presently, there are teams comprised of professional, allied health, and administrative support personnel to enhance the development of a sustained partnership with patients and their caregivers. 

Primary Care Teams follow their patients throughout other outpatient specialty clinics and are alerted to any inpatient admissions to ensure continuity of care issues are addressed. The teams work closely with other disciplines, such as Dietetics, Pharmacy, and Social Work to ensure the patient and caregiver are supported to meet the medical and psychosocial needs of the patient. PCTs focus on education, health promotion, disease prevention and management of acute and chronic conditions.
